Alumni of CICS Chicago Quest, CICS Larry Hawkins, CICS Longwood High School, CICS Northtown Academy, and CICS Ralph Ellison Academy may request an official student transcript using the form below. Please complete all required fields and submit any necessary authorization documents depending on who is making the request.
Important Information
Former students should complete the transcript request form and provide an electronic signature.
Institutions requesting a transcript on behalf of a former student must upload documentation showing the student has authorized the release of their records.
Requests for CICS Longwood, CICS Northtown, and CICS Ralph Ellison are automatically forwarded to that campus's records team for processing.
Processing time is typically 5–7 business days.
